無眼球症における義眼装用の工夫について

New Method of Nonoperative Dilatation Technique of Socket for Congenital Anophthalmos

I．緒言

　先天性無眼球症および小眼球症については，古くより数多くの報告がみられるが，その形成治療に関する報告は少ない。多くの場合，ごく普通の義眼を装用するか^1），義眼の装用も困難な小結膜嚢に対しては，初めより結膜嚢形成術が行なわれている。最近秋山氏はシリコン製のSelf Dilatorを考案し，これを用いてsocketの再建を図り，好結果を得た^2）。

　著者は今回，非常に小さい結膜嚢を有した5カ月の片側性先天性無眼球症の男児に，手術的操作を加えることなく，この秋山氏考案^2）によるシリコン製のSelf Dilator （袋型義眼＋パンタグラフ型圧迫体）を装用し，袋内のシリコンオイルの量を加減することにより結膜嚢の拡張を図り，かなりの効果を得たので報告する。

The dilatation technique of the conjunctival sac was performed on a case of unilateral con-genital anophthalmos in a 5-month-old boy.

The very small conjunctival sac was succes-sfully dilated by a silicon-made dilator. This apparatus consisted of two parts : one was aeel-astic bag-like prosthesis and the other was a pantagraph-shaped apparatus for compression. In the former, silicon oil was contained and the size of the prosthesis could be changed by increasing or decreasing the quantity of silicon oil. The latter served for prevention of falling off the prosthesis and supported its dilation effect.
