網膜剥離に対する鞏膜切除短縮術について

Studies on scleral resection for retinal detachment

Ⅰ．前言

　「自発網膜剥離に対する療法のorthodoxyは，あくまでも裂孔閉塞ジアテルミー法である。」この自明の言葉を敢えて先ず冒頭に述べて，私の話を進めたいと思う。

　この裂孔閉塞ジアテルミー法——以下単にD法と略記する——を補足・補強して，剥離網膜の復位を促進しようとする手段のうちで，綱膜よりも外側から作用させる方式の現況は，第1表に示すようであつて，理念の上から「鞏膜を短縮するグループ」と，「眼球壁を押し凹ませるグループ」とに分けられ，それぞれに多くの種目がみられる。しかし何れにも共通する点は，剥離網膜に脈絡膜を接近させようとの努力である。又，この2つの理念を併せ有する術式もあつて，例えばSchepens-Okamuraのポリエチレン管縫着法の如きである。

In order to consider indications for scleral resection, the author statistically observed 69 eyes with retinal detachment. The above mentioned procedure was given to these 69 eyes out of 278 eyes of 263 inpatients with retinal detachment in Dept. of Ophthalmology, Hiroshima University Hospital during the period between Dec. 1953 and Aug. 1959. (68 mon-ths; 1 month is not included because the hospital was closed for transfer during a whole month of Sept. 1957)

As the factors in each case which are regarded in the relation to retinal reattachment, the author checked up the following items: 1) No or undiscovered retinal tear
