全眼球炎の化学療法，特に抗生剤大量投与療法—Carbenicillin大量投与による眼内移行ならびに臨床効果

Chemotherapy in Panophthalmitis, Ocular penetration and clinical evaluation of Carbenicillin with high dosage

緒言

　全眼球炎は重篤な眼感染症で，なかでも緑膿菌によるものの予後は不良である^6）。

　近年，広域合成ペニシリン剤の一つであるCarbenicillinの大量投与によるグラム陰性桿菌，特に緑膿菌感染症に対する臨床効果がクローズアップされてきている。全眼球炎に対しても当然その適応が考えられて臨床効果が期待されるところである。

Fundamental and clinical experiments for chemotherapy with high dosage of Carbenicillin (CBPC) against panophthalmitis were performedand the results summarized as follows.

1. Ocular penetration of CBPC in rabbit eyes

The ocular penetrations of CBPC were exa-mined following intravenous injection of 50mg/ kg, as usual dose, and 250mg/kg, as high dose.

1) The ocular tissue levels increased with application of higher dosage, 250mg/kg. The aqueous humor ratio to serum levels was higher in 250mg/kg injection than 50mg/kg injection.

2) The half life of CBPC in the aqueous humor prolonged with injection of 250mg/kg dosage.
