A case of 4-year-old girl is reported. She was referred to us with left exophthalmos as the sole complaint. Increase of exophthalmos caused the eyeball to deviate inferotemporally with impairment of visual acuity. Roentgeno-gram revealed thinning of the upper-nasal rim of the orbit. Killian's operation, performed ten months after the first visit of the patient, prov-ed the lesion to be frontal pyocele.

Two main causes of unilateral exophalmos are tumor and inflammation. Tumor is more fre-quent in children than in adults and vice versa as to inflammation.
