人眼Schlemm氏管手術を修得するための段階的トレーニングについて

Stepweise Training for the Operation of Schlemm's Canal of the Human Eye

緒言

　近年Schlemm氏管（以下Sと略）を中心とする緑内障手術が盛んに行なわれるようになつてきているが，手術顕微鏡の性能が向上しても，Sを確実に扱うことは容易ではない。動物および屍体眼についての経験や，電子顕微鏡レベルでの知識もただちに生体人眼に応用できるわけではない。誰にとつても，人眼Sの発見およびとり扱いに苦労するという時期が存在する。術者の腕の修練のために患者の眼が犠牲になることは許されない。人眼を土台に，術者も手術顕微鏡のレベルでSについての知識をえられると同時に，患者にとつては余計な侵襲とならない方法を見出すことは，特に医学教育の上で重要な問題である。ここに私達の考察を述べる。

Training for getting used to the manipulati-on of Schlemm's canal in the human eye was divided into three stages. In the first stage, by making use of Schlemm's canal in the scleral piece resected on trabeculectomy ab externo, the operator is accustomed to Schlemm's canal itself. In the second stage, he confirms his ab-ility of finding Schlemm's canal from the out-side by using nylon filament in Smith's meth-od. In the third stage, he advances to Harms-Dannheim's method using the knowledge obta-ined so far to operate Schlemm's canal on a full scale.
