緒言

　Trabeculotomy ab externoを行なつた者であれば誰しも，直視下でまさしくSchlemm管（以後S管と略）を確認した（と思つた）にもかかわらず，術後の隅角検査で違う所が開放されているのを発見して愕然とした経験を持つているであろう。技術の未熟といえばそれまでであるが，手術顕微鏡の倍率を30倍位に上げても，時にこのようなことが生じるのには，他の手術に比して間違いやすいなにかがあるに違いない。私たちは今までの経験から，一見まさしくS管のように見える状態が，一寸した術者の誤つた先入感によつて顕微鏡下でも意外に簡単に作り出されることを見出した。この「ニセのS管」がどのようにして現われてくるのか，私たちの考えを述べたい。

Exact location of Schlemm's canal during trabeculotomy ab externo under operating mic-roscope is not always easy. It sometimes occurs that what appears to be the Schlemm's canal is actually a false one. This is prone to occur when lamellar resection of the sclera proceeds obliquely as this may produce a false scleral spur which is the landmark for locating the canal. Another cause for false localization of the canal may lie in the mistaken belief that oozing of aqueous humor takes place only when the Schlemm's canal is opened.
