白内障顕微鏡下嚢外摘出術の新手術法

A New Method for Microsurgery of Extracapsular Cataract Extraction

緒言

　最近は手術用顕微鏡の発達，冷凍摘出術の応用，手術器具や縫合材料等の著しい改良により，比較的若年者でも白内障の嚢内摘出術が広く行なわれるようになつたが，硝子体脱出やそれに引き続く各種の障害，pupillary block等の術中，術後合併症を防ぐ安全性という点から，嚢外摘出術の価値は少しも衰えていない。ただ従来の嚢外摘出術では術後，後発白内障の発生が通常不可避であり，その切裂術等の手術がさらに1〜2回必要となる等の欠点があつた。そこで従来の嚢外摘出術の欠点を除き，嚢内摘出術に伴う各種の合併症がなく，両者の長所を兼ね備えた新しい顕微鏡下嚢外摘出術を開発した。

　本手術法は私どもが確認した先天性および後天性白内障の大部分において，水晶体嚢は混濁せず完全な透明性を維持しており，嚢外摘出術後も後嚢の透明性は完全かつ永続的に保たれるという事実に基づいている。

A new ophthalmic surgery for extracapsu-lar cataract extraction was performed on com-plicated cataract, congenital cataract and senile cataract. This surgery has no faults of extra-capsular extraction and no complications which accompany intracapsular extraction.

1. This surgery was developed based on the following facts.

1) The lens capsule maintains completely its transparency in most of congenital and acquired cataract cases.

2) Especially, the posterior capsule keeps its transparency perfectly and continuously even after extracapsular extraction.

2. A new capsulotome for this surgery was made by authors.
