3％ N-Acetyl cysteine点眼液の種々なる角膜疾患，特に単純ヘルペス性角膜炎に対する臨床的効果について

Clinical Value of Acetyl Cysteine Eyedrops for Keratopathies Including Herpes Simplex Keratitis

緒言

　潰瘍性角膜疾患は一般に難治であり，近年col—lagenase inhibitorの効果に期待が寄せられている。なかでも単純ヘルペス性角膜炎（以下HS—Kと略）は発生頻度も高く，いまだ確実な治療法もなく，角膜代謝の崩壊による不透明化のため，治療薬であるIDUのある現在でも失明例があとをたたない。これまで著者らはcollagenaseinhibitorとしてcysteineを含有している強力ネオミノファーゲンC注射液（以下強ミノCと略）^1）をそのまま点眼薬として使用していたが，cysteineそのものが酸化されやすく，その薬理効果を長く保つことができないため，期待するほどの十分な治療効果は得られなかつた。今回，著者らはcollagenase阻害効果，安定性共にすぐれた3％　N-Acetyl cysteine点眼液（以下AC—ETと略）を試用する機会を得，HSKその他の角膜疾患に応用したとしろ，満足すべさ治療効果を得たので報告する。

The collagenase activity in the cornea is re-portedly increased when corneal ulcers develop and progress. Topically applied collagenase in-hibitors promise to be effective for treatment of corneal ulcers. We applied 3% aqueous so-lution of N-acetyl cysteine, one of collagenase inhibitors, to the treatment of herpes simplex keratitis (11 eyes), corneal ulcers which deve-loped as complication of vernal catarrah (2), keratitis caused by burn (1), parencymatous keratitis (1), corneal erosion caused by chem-ical substance (1), bullous keratitis (1) and keratoconjunctivitis sicca (1). Instillation 6 ti-mes a day resulted in satisfactory beneficial effects in all the cases. The response was pa-rticularly favorable in herpes simplex keratitis. The treatment was also effective for disorders of the corneal stroma without ulcer or epithe-lial defects.
