白内障顕微鏡下嚢外摘出術の再検討

Extracapsular cataract extraction under the microscope. A review

緒　言

　白内障状態においても水晶体嚢は混濁せず完全な透明性を維持しており，嚢外摘出術後も残された後嚢の透明性は完全かつ永続的に保たれる。私の確認したこの事実に基づいた新しい顕微鏡下嚢外摘出術を発表^1）して以来4年が経過した。その後も乳児より高齢者に至る多数の白内障手術に本手術法を施行し，本法の安全性と確実性を更に確めえたが，その間の経験からなお若干の改良を行い，本法を行うにあたつて注意すべきいくつかの知見も得たので報告する。ただし手術についての基本的な点については何らの変更はない。

Disposable 1/2 inch 26-gauge needles are suita-ble for cutting of the anterior lens capsule. The anterior lens capsule is resected along the pupi-nary margin circularly under full mydriasis with the disposable needle which is inserted into the anterior chamber through the limbus. After co-mpletion of the corneoscleral section, the main part of the lens substance is isolated from the posterior capsule as a lump and is easily removed.The residual lens substance is then removed by aspiration with alternate irrigation and suction using 1/4, 1/3 and 1/2mm needles.
