前眼部OCTを用いた角膜潰瘍の観察

Findings of corneal ulcer observed by anterior segment optical coherence tomograph

要約　目的：角膜潰瘍2症例の，前眼部光干渉断層計（OCT）による経時的な所見の報告。症例と経過：1例は26歳女性で，2年前から睫毛乱生による角膜浸潤でステロイド薬の点眼をしていた。1週間前から左眼に疼痛が生じた。左眼の角膜中央部に潰瘍があり，Descemet膜皺襞があった。潰瘍部の角膜厚は180μmで，真菌性潰瘍が疑われた。薬物投与と治療用コンタクトレンズの装用を行い，1か月後に寛解した。他の1例はCrohn病の既往がある64歳男性で，左眼の眼痛と視力低下で受診した。左眼の角膜耳下側に辺縁部潰瘍があり，潰瘍がある部位の角膜厚は320μmであった。薬物投与と治療用コンタクトレンズの装用を行い，16日後に潰瘍は寛解した。2症例とも，前眼部OCTにより，治療用コンタクトレンズの装用で角膜上皮が進展し，角膜実質の再生と角膜厚の増加が観察された。結論：前眼部OCTにより，治療用コンタクトレンズを装用した角膜潰瘍の治癒過程が観察された。

Abstract.　Purpose：To report findings in 2 cases of corneal ulcer followed up using an anterior segment optical coherence tomograph（OCT）. Cases and Findings：One was a 26-year-old female who presented with pain in the left eye since one week. She had been treated by topical corticosteroid for corneal infiltration due to trichiasis. The left eye showed corneal ulcer with corneal thickness of 180μm. She was treated by medication and therapeutic contact lens for suspected fungus infection. The corneal lesion resolved one month later. The other case was a 64-year-old male with pain and blurring in the left eye. He had a history of Crohn disease. The left cornea showed marginal ulcer with corneal thickness of 320μm. Medication and wear of contact lens was followed by resolution 16 days later. In both cases, anterior segment OCT showed proliferation of corneal epithelial cells to cover the ulcer with increase in corneal thickness. Conclusion：Anterior segment OCT was useful in evaluating the course of recovery of corneal ulcer.
