両眼多焦点眼内レンズ挿入眼に起きた片眼外傷性無虹彩症の1例

Unilateral traumatic aniridia in a case implanted with multifocal intraocular lens in both eyes

要約　目的：鈍性外傷で，多焦点眼内レンズ挿入眼の虹彩が脱出し，眼内レンズ（IOL）には脱出や偏位がなかった症例の報告。症例：69歳男性が，右眼に打撲を受けた翌日に受診した。2年前に白内障手術と多焦点IOL挿入術を両眼に受けていた。所見：矯正視力は右光覚弁，左1.2で，右眼には前房出血があり，無虹彩であった。IOLは囊内に残り，偏位はなかった。前房洗浄と硝子体手術を行い，3か月後に遠見視力は1.2，裸眼での近見視力は0.6に回復した。結論：無虹彩であっても，多焦点IOL挿入眼の遠見視力は回復し，矯正の有無にかかわらず近見視力は不十分であった。

Abstract.　Purpose：To report a case who had multifocal intraocular lens in both eyes and who developed unilateral aniridia after blunt trauma. Case：A 69-year-old male presented with blunt trauma to the right eye the previous day. He had received implantation of multifocal intraocular lens in both eyes 2 years before. Findings：Corrected visual acuity was light perception right and 1.2 left. The right eye showed hyphema with total aniridia. The intraocular lens（IOL）remained in the bag without displacement. The right eye was treated by vitectomy and irrigation of the anterior chamber. Unaided visual acuity improved to 1.2 for distance and 0.6 for near 3 months later. Conclusion：In spite of total aniridia, visual acuity recovered for distance and partially for near.
