β-ラクタマーゼ非産生アンピシリン耐性インフルエンザ菌による涙腺導管炎が疑われた1例

A case suspected of meibomitis due to β-lactamase nonproducing ampicillin-resistant Haemophilus influenzae

要約　目的：β-ラクタマーゼ非産生アンピシリン耐性インフルエンザ菌（BLNAR）による涙腺導管炎が疑われた1例を報告する。

症例：41歳，女性。右眼の充血と眼脂を主訴に，数か所の眼科を受診し，複数の抗菌薬点眼を使用したが症状の改善がなく，難治性結膜炎として川崎市立多摩病院を紹介され受診した。初診時の眼脂培養検査でBLNARが検出され，薬剤感受性試験結果に基づく抗菌薬点眼を開始したが，眼脂が継続した。その後，右外眼角部結膜に排膿する瘻孔を伴う炎症性の肉芽腫性隆起病変を認め，睫毛の迷入などによる涙腺導管炎が疑われたため，結膜を切開し，内容物の搔爬を行った。内容物は黄色調の粥状物が排出され，睫毛などの異物はなかった。病理像はわずかに炎症細胞を認めたが，大部分は硝子様物質で，細菌培養検査は陰性であった。術後は外眼角部に肉芽組織が残存したが，眼脂は速やかに改善し，再発はなかった。

結論：涙腺導管開口部から侵入したBLNARによる涙腺導管炎と，結膜下膿瘍形成の可能性が疑われた。診断は細菌培養検査や薬剤感受性試験が有用で，これに基づく適正な抗菌薬使用が重要であるが，薬物治療で完治しない際は手術を検討する必要がある。

Abstract　Purpose：To report a case suspected of meibomitis due to β-lactamase nonproducing ampicillin-resistant Haemophilus influenzae（BLNAR）.

Case：A 41-year-old female was referred to us as refractory conjunctivitis. She showed hyperemia and eye discharge of the right eye as the main complaint. She had been treated with eyedrops at multiple clinics but the symptoms had persisted.

Findings and Clinical Course：Her right eye showed hyperemia and eye discharge. BLNAR was detected by culture of the discharge. The right eye was treated by gatifloxacin eyedrops and ofloxasin ointment. Eye discharge persisted and she was suspected of meibomitis in the right eye. The content was curetted about 8 months after onset of symptoms. The yellowish content was removed and contained no foreign body. Pathological specimen contained a few inflammatory cells. Bacterial culture was negative. The symptoms improved promptly with no recurrence.

Conclusion：This case is suspected of meibmitis with abscess formation llustrates that caused by BLNAR invading through the opening of meibomian gland. Bacterial culture was useful for the diagnosis and treatment. Surgery became necessary after drug treatment proved futile.
