Abstract Purpose:To investigate changes in the clinical manifestations over time of adenovirus(Adv)conjunctivitis.

Patients and methods:A total of 213 patients(349 eyes)who visited a clinic in Tokyo from 2014 to 2019 and who tested positive for Adv by a rapid diagnostic kit were included. Items such as clinical symptoms, drugs used, multiple subepithelial corneal infiltrates(MSI), and occurrence of conjunctival pseudomembrane(PM)were retrospectively examined.

Results:The number of Adv conjunctivitis cases by age group was 18(28 eyes)in 2014, 42(70 eyes)in 2015, 49(79 eyes)in 2016, 30(49 eyes)in 2017, 42(71 eyes)in 2018, and 32(52 eyes)in 2019. Although the incidence of MSI was 0% in 2014, it was 18.3% overall. The incidence of conjunctival PM was 14.3% in 2014 and 12.9% in 2015;however, it remained in the single digits thereafter, and was 6.6% overall. In bilateral conjunctivitis, the incidence of MSI was 21.7%, which was significantly higher than that in unilateral one(p=0.041), while that of conjunctival PM was also significantly higher in bilateral conjunctivitis at 8.1%(p=0.0034). The incidence of MSI was 30.2% in the fluorometholone eye drop group and 10.5% in the betamethasone eye drop group, showing significant difference(p<0.001). The incidence of PM was 6.5% in the fluorometholone group and 10.1% in the betamethasone group, with no significant difference(p=0.397).

Conclusion:The incidence of complications from Adv conjunctivitis varies greatly depending on the type of epidemic. However, attention should be paid to the occurrence of MSI when conjunctivitis occurs in bilateral eyes. However, the use of steroid eye drops requires careful consideration depending on the epidemic type and clinical symptoms.
