要約　目的：複視を初発症状とした悪性リンパ腫によるGarcin症候群の症例の報告。症例：70歳男性が40日前からの複視と左眼視力低下で受診した。所見と経過：矯正視力は右1.5，左0.6で，眼底に異常がなく，左眼に眼瞼下垂と全方向への運動制限があった。嚥下障害があり，左側の第3，4，6，9，10，12脳神経の障害があった。全身検査で悪性リンパ腫と診断された。磁気共鳴血管造影（MRI）で頭蓋底に腫瘍が疑われた。初診の11日後に呼吸障害で不帰の転帰をとった。結論：複視を初発症状としてGarcin症候群が発症することがある。

Abstract.　Purpose：To report a case of Garcin syndrome with diplopia as the initial manifestation. Case：A 70-year-old male presented with diplopia and impaired visual acuity in the left eye since 40 days before. Findings：Corrected visual acuity was 1.5 right and 0.6 left. Funduscopy showed normal findings. The left eye showed blepharoptosis and restricted movement in all directions. He had difficulty in swelling. He showed eventually palsy of Ⅲ, Ⅳ, Ⅵ, Ⅸ, Ⅹ and Ⅻ cranial nerves on the left side. He was diagnosed with malignant lymphoma. Magnetic resonance imaging（MRI）showed findings suggestive of invasion of tumor in the skull base. He passed away 11 days after his initial visit due to respiratory difficulties. Conclusion：This case illustrates that diplopia may be the initial manifestation of Garcin syndrome.
