強い眼症状により診断された月経前症候群の1例

A case with severe ocular manifestations leading to the diagnosis of premenstrual syndrome

要約　目的：強い眼症状の訴えにより眼科を受診し，月経前症候群（PMS）の診断に至った症例の報告。症例：34歳女性。約2年前からの冬季に限定した隔月で月経周期に伴って起こる右眼痛で受診した。20歳時に子宮外妊娠で片側卵管の摘出術を受けた既往と，受診後の検査で松果体囊腫の指摘がある。本症例は，鑑別診断およびPMS診断基準によってPMSと判断された。冬季および隔月に発症する原因としては，それぞれ松果体囊腫と卵管摘出の関与を推定した。結論：眼症状が著明なPMSがみられることがある。

Abstract.　Purpose：To report a case with severe ocular symptoms who was finally diagnosed with premenstrual syndrome. Case：A 34-year-old female presented with hyperemia in the right eye since 2 years before. The hyperemia developed every two months just before menstruation and only in winter. She had received unilateral salpingectomy for extrauterine pregnancy at the age of 20 years. She had given birth to 2 children at the age of 22 and 30 years respectively. Findings：Magnetic resonance imaging（MRI）showed pineal cystoma. She was diagnosed with premenstrual syndrome according to the diagnostic criteria for the condition. There is a high possibility that prior salpingectomy and pineal cystoma may be related with the bimonthly occurrence of ocular symptoms and only in winter. Conclusion：This case illustrates that premenstrual syndrome may manifest as ocular symptoms.
