白内障と硝子体の同時手術を施行したハンセン病の糖尿病網膜症の1例

Simultaneous pars plana vitrectomy, lensectomy and intraocular lens implantation in a case of leprosy and diabetic retinopathy

（P1-1-15）　ハンセン病の白内障・糖尿病綱膜症に対し白内障と硝子体の同時手術（トリプル手術）を行った1例を経験した。症例は68歳男性で，主訴は右眼視力低下。既往歴に16歳頃からハンセン病（BL型），60歳から糖尿病がある。初診時の右眼視力は0.03で，白内障と糖尿病網膜症による硝子体出血がみられた。トリプル手術の後，軽度の虹彩炎がみられたが速やかに消退し．右視力は0.06まで改善した。本症例は術前後においてハンセン病に起因するぶどう膜炎の合併がみられず，比較的侵襲の高いトリプル手術を行ったが良好な結果が得られた。

A 68-year-old male presented with blurring of vision in the right eye and bilateral lagophthalmos. He had been diagnosed as leprosy type BL at the age of 16. Diabetes mellitus was detected 8 years before. The visual acuity was 0.03 right, apparently due to cataract and vitreous hemorrhage. The right eye was treated by simultaneous pars plana vitrectomy, lensectomy and intraocular lens implantation.Postoperative iritis was temporary and the visual acuity improved to 0.06.This case illustrates that our triple surgery was effective with minimum leprotic uveitis before or after surgery.
