要約　目的：単独または硝子体手術と同時に行う白内障手術で，シングルピースアクリルレンズ用に設定されたIOL Master^TMのA定数の検討。対象と方法：眼内レンズとしてSN60WF（Alcon社）を挿入した94眼を対象とした。60眼には白内障単独手術，34眼には硝子体手術を同時に行った。34眼中9眼にはガスを注入した。眼軸長をIOL Master^TMで測定し，A定数を119.2に設定し，SRK/T式で度数を算出した。目標度数からのずれを術後3か月まで測定した。結果：白内障単独手術群では術後1日目に若干の近視化が生じ，最終的には目標屈折度数に近似した。硝子体同時手術群では約0.5Dの近視化があった。両群とも術後屈折は全期間を通じ安定していた。結論：SN60WFに対するIOL Master^TM用のA定数119.2は適正である。硝子体手術を同時に行う場合には，近視化があるのでA定数の変更が必要である。

Abstract.　Purpose：To evaluate the constant A for single-piece acrylic lens in cataract surgery with or without vitrectomy using IOL Master^TM. Cases and Method：This study was made on 94 eyes that received implantation of SN60WF（Alcon,Inc）. Cataract surgery alone was performed on 60 eyes and with vitrectomy in 34 eyes,nine of which received vireous-gas exchange. Axial length was measured by IOL Master^TM. Power of intraocular lens was calculated by SRK/T formula by setting the constant A at 119.2. Cases were followed up for 3 months. Results：Postoperative refraction was close to intended value after cataract surgery alone. It shifted towards myopia by 0.5 diopters after simultaneous vitreous surgery. Conclusion：The value of 119.2 as constant A is appropriate for SN60WF and IOL Master^TM after cataract surgery alone. Modification of constant A is needed for simultaneous vitrectomy because of myopic shift after surgery.
