要約　目的：僚眼が失明しているハンセン病患者に行った白内障手術の成績の報告。対象と方法：僚眼が失明しているハンセン病患者8例8眼に白内障手術として，超音波水晶体乳化吸引術と眼内レンズ挿入術を行った。全例ぶどう膜炎の既往があり僚眼の視力は全例が手動弁以下で，5眼には光覚がなかった。術後20～157か月（平均104か月）の経過を追った。結果：術後の最高視力は全例で4段階以上改善し，0.5以上になった。最終視力は5眼で2段階以上改善し，3眼が0.5以上になった。視力が0.5未満の原因は，角膜障害と黄斑変性であった。結論：僚眼が失明して，ぶどう膜炎の既往があっても，ハンセン病患者での白内障手術の短期成績は良好であった。長期観察では視力が低下する症例があり，ぶどう膜炎以外の眼疾患にも注意する必要がある。

Abstract.　Purpose：To report the outcome of cataract surgery in patients with the fellow eye lost due to leprosy. Cases and Method：This retrospective study was made on 8 patients of leprosy who received phacoemulsification-aspiration with intraocular lens implantation for cataract in one eye. All had a history of uveitis and visual acuity less than hand motion in the fellow eye. Five eyes had no light perception. All the cases were followed up for 20 to 157 months,average 104 months,after surgery. Results：All the 8 eyes showed best visual acuity of 0.5 or over. Final visual acuity was better by 2 lines or more in 5 eyes. Three eyes had final visual acuity of 0.5 or over. Visual acuity less than 0.5 was due to keratopathy or macular degeneration. Conclusion：Even when the fellow eye had been lost,cataract surgery was safe and effective for the other eye in leprosy patients with uveitis. Postoperative visual acuity may deteriorate on long-term basis due to ocular lesions other than uveitis.
