ハンセン病性ぶどう膜炎既往者の白内障術後炎症再燃と抗lipoarabinomannan-B抗体

Recurrence of uveitis and lipoarabinomannan-B antibody after cataract surgery in patients of Hansen's disease

要約　目的：ハンセン病によるぶどう膜炎の白内障術後の再燃と，抗酸菌の共通抗原であるlipoarabinomannan-B（LAM-B）に対する抗体値との関連の報告。対象と方法：ハンセン病によるぶどう膜炎の既往があり，1995年7月までの17か月間に同一術者が白内障手術を行った症例のうち，5年以上の術後観察ができ，術前のらい菌検査が10年以上陰性で，術前に抗LAM-B抗体価が検索できた13例16眼を対象とした。結果：前房内炎症が5例6眼で再燃した。うち4例5眼では抗LAM-B IgGとIgM抗体価がともに陽性であった。炎症の再燃がない8例10眼では，両抗体価がともに陽性の例はなかった。結論：ハンセン病によるぶどう膜炎の既往があり，抗LAM-B IgGとIgM抗体価がともに陽性である症例では，白内障術後にぶどう膜炎が再燃する可能性が高く，長期間の経過観察が必要である。

Abstract.　Purpose：To report the correlation between recurrence of uveitis after cataract surgery and antibody titer to lipoarabinomannan-B（LAM-B）in patients with Hansen disease. Cases and Method：This study was made on 16 eyes of 13 patients who received surgery during 17 months through July 1995 by a single surgeon. They were followed up for 5 years or longer. They were negative for bacterial of Hansen's disease over 10 years. They were checked for LAM-B before cataract surgery. Results：Anterior uveitis recurred in 6 eyes of 5 patients. Anti-LAM-B IgG and IgM were both positive in 5 eyes of 4 patients. These antibody titers negative in 10 eyes of 8 cases who showed no recurrence of uveitis. Conclusion：There is a high risk of recurrence of uveitis after cataract surgery in cases of Hansen's disease with elevated anti-LAM-B IgG and IgM.
