要約　目的：梅毒性眼感染症を契機としてヒト免疫不全ウイルス（HIV）感染が発見された症例の報告。症例と経過：37歳男性が急激な左眼疼痛と視力低下で受診。5か月前から両眼の羞明と右眼視力低下があった。矯正視力は右0.3，左手動弁であり，両眼に虹彩炎，硝子体混濁，視神経炎，網脈絡膜炎がみられた。梅毒血清反応が強陽性であり，梅毒性ぶどう膜炎ならびに視神経炎と診断し，駆梅療法と副腎皮質ステロイド薬の治療により，視神経炎とぶどう膜炎は軽快した。全身の精査でHIV感染があることが判明した。初診から2か月後の矯正視力は右0.5，左0.7であった。結論：梅毒による眼感染症では，HIVその他の性感染症が併発している可能性に留意すべきである。

Abstract.　Purpose：To report a case of ocular syphilis associated with human immunodeficiency virus（HIV）infection. Case and Findings：A 37-year-old male presented with acute pain and blurring in his left eye. He had photophobia and impaired visual acuity since 5 months before. His corrected visual acuity was 0.3 right and hand motion left. Both eyes showed iritis, vitreous opacity, retinochoroiditis, and optic neuritis. Serological test was strongly positive for syphilis, leading to the diagnosis of syphilitic uveitis and optic neuritis. Systemic studies showed him to be positive for antibody to HIV. Treatment with antibiotics and corticosteroid was followed by improved uveitis and optic neuritis, with visual acuity of 0.5 right and 0.7 left 2 months later. Conclusion：This case illustrates that ocular syphilis may be associated with HIV infection.
