特発性黄斑円孔の術前・術後の固視点

The point of fixation in idiopathic macular holes before and after surgery

　特発性黄斑円孔30眼の硝子体手術前後における固視点の分布，および手術前後での固視点の変化を検索した。術前に検査した14眼は全例で，術後では23眼中21眼で固視点は中心窩もしくは上方1／2の円孔縁に分布していた。術前後で検査できた6眼では，術前後で固視点の移動はなかった。また，術後の中心窩固視の有無，および円孔底の消失の有無と術後視力との関係を統計学的に検討したところ，中心窩固視例，円孔底消失例では有意に良好な術後視力が得られた。以上のことから，黄斑円孔の固視点は黄斑の上方1／2に分布する傾向にあること，また，術後円孔底が消失すると中心窩固視が可能となり，その場合の視力は良好であることが明らかとなった。

We examined the point of fixation in 30 eyes of idiopathic macular hole before and after successful vitreous surgery. The point of fixation was located either within the fovea or along the superior half of the edge of the hole in all of 14 eyes prior to surgery and in 21 of 23 eyes after surgery. There was no shift in the point of fixation in 6 eyes before and after surgery. There was a significant improvement in postoperative visual acuity in eyes with foveolar fixation and in eyes in which the macular hole had closed after surgery. The findings show that the point of fixation is often distributed along the superior edge of the macular hole and that the visual outcome is correlated with the anatomical recovery and point of fixation after surgery.
