We examined the point of fixation in 30 eyes of idiopathic macular hole before and after successful vitreous surgery. The point of fixation was located either within the fovea or along the superior half of the edge of the hole in all of 14 eyes prior to surgery and in 21 of 23 eyes after surgery. There was no shift in the point of fixation in 6 eyes before and after surgery. There was a significant improvement in postoperative visual acuity in eyes with foveolar fixation and in eyes in which the macular hole had closed after surgery. The findings show that the point of fixation is often distributed along the superior edge of the macular hole and that the visual outcome is correlated with the anatomical recovery and point of fixation after surgery.
