Abstract. Purpose:To report a case of Behçet disease manifesting anterior ischemic optic neuropathy. Case:A 53-year-old male had been diagnosed with Behçet disease 7 years before. He had aphthous stomatitis, erythema nodosum, genital ulcer, and bilateral iridocyclitis. He had had corrected visual acuity of 1.5 in either eye. Clinical symptoms had improved following systemic colchicine and prednisolone. He developed pneumonia 2 months before, followed by pustulosis palmaris and plantaris. His left visual acuity decreased to 0.01. The left eye showed swollen optic disc and constricted visual field. Fluorescein angiography showed findings consistent with anterior ischemic optic neuropathy. He received pulsed corticosteroid therapy followed by peroral aspirin and cyclosporin. Nine months after the episode, the optic disc has become atrophic with visual acuity of 0.3 and horizontal hemianopia. Conclusion:Anterior ischemic optic neuropathy may be involved in Behçet disease with sudden visual disturbance.
