Abstract. Purpose:To report a case of keratomycosis with corneal perforation caused by Paecilomyces species. Case:A 78-year-old male presented with hyperemia since 4 days before. He had been treated for lagophthalmos during the foregoing 8 years. Findings:His corrected visual acuity was hand motion right and 0.9 left. The right eye showed ciliary injection, hypopyon, and corneal ulcer in the lower quadrant. Filamentous fungus was present in the corneal scrapings. It was identified as Paecilomyces lilacinus by culture. The lesion failed to respond to topical and systemic fluconazole with topical pimaricin followed by systemic voriconazole. The cornea perforated on day 18 of disease. The lesion subsided after covering by conjunctiva and topical voriconazole. Conclusion:This case illustrates that keratomycosis by Paecilomyces species may run a fulminant course. Early diagnosis and treatment are advocated.
