Behçet病における新規発症患者の動向

Current status of new occurrence of Behçet disease

要約　目的：秋田大学眼科でのBehçet病の初診患者の動向の報告。対象と方法：2002年までの5年間に当科外来を初診したBehçet病患者を診療録の記録に基づいて検討した。その結果を1982年までの11年間と1997年までの5年間の当科での発症状況と比較した。結果：この5年間に12例がBehçet病と診断され，うち7例に眼症状があった。その内訳は男性5例，女性2例であり，年齢は平均27.0±8.8歳であった。眼症状がある7例では治療と経過観察を行った。最終受診時に13眼中6眼で視力が0.1以下であり，すべて黄斑変性と視神経萎縮がその原因であった。ぶどう膜炎のなかでBehçet病が占める頻度は，1982年までの11年間では31.9％，1997年までの5年間では5.3％，今回の5年間では3.0％であり，新規に発症する患者数は減少していた。結論：1982年までの期間と比較し，2002年までの5年間の新規発症は有意に減少している。この事実は，わが国でのBehçet病の新規発症が減少しつつあるという従来の報告と一致する。

Abstract.　Purpose：To report the incidence of Behçet disease among new patients seen at the Akita University Hospital. Cases and Method：This retrospective study was made on patients of Behçet disease seen by us during 5 years through 2002. The findings were compared with our previous reports during 1972-1982 and during 1993-1997. Results：There were 12 new patients diagnosed with Behçet disease. The eye was involved in 7 patients, comprising 5 males and 2 females. Their age averaged 27.0±8.8 years. At their last visit, visual acuity was below 0.1 in 6 out of 13 eyes. The visual failure was due to macular and optic atrophy. Incidence of Behçet disease among uveitis as a whole was 31.9％ during 11 years through 1982, 5.3％ during 5 years through 1997, and 3.0％ during 5 years through 2002. Conclusion：Incidence of Behçet disease declined significantly during 5 years through 2002 when compared with 11 years through 1982. This finding agrees with past reports regarding new occurrence of Behçet disease in Japan.
