間質性腎炎患者に発症した進行性網膜外層壊死の1例

Progressive outer retinal necrosis in a patient with interstitial nephritis

目的：間質性腎炎患者に発症した進行性網膜外層壊死の報告。症例と経過：68歳男性が2年前に間質性腎炎と診断され，副腎皮質ステロイドの全身投与を受けていた。1か月前に帯状疱疹が頭部と足に発症した。2週前からの左眼視力低下と両眼の視野狭窄で受診した。矯正視力は右1.2，左0.02であり，前房と硝子体の炎症所見は乏しかった。両眼の眼底に黄白色滲出斑が散在し，網膜出血や血管炎は軽度であった。前房水から水痘・帯状疱疹ウイルス（varicella zoster virus）が検出された。眼底所見から病変は網膜外層にあると推定し，進行性網膜外層壊死と診断した。アシクロビルとガンシクロビルを全身的に投与したが病変は急速に進行し，両眼とも失明した。発症の翌年，敗血症と多臓器不全で死亡した。結論：本症例は間質性腎炎に対する副腎皮質ステロイドの全身投与で低免疫状態にあったと推定され，進行性網膜外層壊死の視力と生命予後が不良であった。本症の迅速な診断と有効な治療法の開発が期待される。

Purpose：To report a case of interstitial nephritis with progressive outer retinal necrosis（PORN）. Case and Clinical Course：A 68-year-old man had been diagnosed with interstitial nephritis 2 years before. He had been treated with systemic corticosteroid. He had developed herpes zoster in the head and foot one month before. He visited us for failing vision in his left eye and bilateral visual field constriction since 2 weeks before. His visual acuity was 1.2 right and 0.02 left. The anterior chamber and vitreous showed minimal signs of inflammation. Both fundus showed exudates. The fundus lesion appeared to be mainly located in deeper retinal layers. Varicella zoster virus was isolated from the aqueous humor,leading to the diagnosis of PORN. The fundus lesion rapidly progressed leading to blindness after systemic aciclovir and ganciclovir. The patient died one year later due to septicemia and multiple organ failure. Conclusion：This case illustrates that visual and life prognosis may be very poor after PORN had set in. There is a keen need for prompt diagnosis and effective therapeutic modality.
