涙囊乳頭腫の1例

A case of papilloma of the lacrimal sac

48歳男性が1年前からの左眼涙囊部の腫脹で受診した。左側の通水テストで逆流があり鼻涙管閉塞が疑われた。頭部CTで涙囊の腫脹があり涙囊炎が推定された。抗菌薬の内服と点眼，NSチューブ挿入でいったん軽快した。4か月後に涙囊が再び腫脹し，左下涙点から腫瘍の一部が突出した。生検で乳頭腫と診断され，涙囊鼻腔吻合術または涙囊摘出術を予定し手術を行った。涙囊が腫瘤で充満していることが術中に確認され涙囊を摘出した。涙囊部腫瘍が涙囊炎様の症状で初発した症例である。

A 48-year-old male presented with swelling of left lacrimal sac area since 1 year before. Water-draining test showed obstructed nasolacrimal passage. Computed tomography（CT）showed swollen lacrimal sac,suggesting acryocystitis. The findings improved after systemic and topical antibiotics and insertion of nunchaku-style tube but recurred 4months later. A part of tumor was found protruding from the left inferior lacrimal punctum. It was identified as papilloma by biopsy. Dacryocystectomy was performed after the lacrimal sac was filled by tumor mass during surgery. This case illustrates that lacrimal sac tumor may manifest symptoms simulating dacryocystitis.
