Abstract. Purpose:To report a bilateral case of progressive outer retinal necrosis(PORN)with retinal detachment. Case:A 47-year-old male presented with constriction of visual field in his right eye since 3 days before. He had been found to be positive to human immunodeficiency virus(HIV)5 months before, followed by varicella zoster virus(VZV)meningitis, toxoplasmic encephalopathy, and cutaneous herpes zoster. His corrected visual acuity was 0.8 right and 1.2 left. His right eye showed yellowish white lesions in the whole circumference and localized retinal detachment. The left fundus showed patchy white lesions in the periphery. Presence of DNA for VZV in the aqueous, led to the diagnosis of PORN. The right eye developed total retinal detachment 16 days later in spite of systemic antiviral therapy. Exudative lesions developed in the left fundus followed by retinal detachment secondary to holes in the necrotic retina. The retina became reattached after surgical interventions including vitrectomy. The left eye has been healed with visual acuity of 0.8 during 6 months until present. Conclusion:PORN is prone to be associated with immunocompromised state, may prove resistant to antiviral or surgical therapy, and may result in poor outcome.
