A 64-year-old man noted inability to recognize human faces including his own in a mirror,or prosopagnosia. He was then diagnosed with bilateral infarction in the occipital lobe. He was seen by us 17months after onset of prosopagnosia. His corrected visual acuity was 0.1p in either eye. Both eyes showed central scotoma within 5 degrees of point of fixation. He still could not identify human faces,but he was capable of recognizing non-human objects. Magnetic resonance imaging(MRI)showed infarction involving bilateral occipitotemporal lobes. His ophthalmological findings remained stationary until his unexpected death 4 years later. This case illustrates that prosopagnosia may be associated with central scotoma as a rare complication.
