要約　目的：両眼の眼内の悪性リンパ腫に対し，メトトレキサートの硝子体注射と予防的髄腔内注射を行った症例の報告。症例：69歳男性が1か月前からの両眼の視力障害で受診した。所見と経過：矯正視力は右0.7，左0.4で，前房には炎症の所見はなく，両眼に硝子体混濁があった。眼底には著変はなかった。2か月後の左眼の硝子体生検でインターロイキン（IL-）6とIL-10の上昇があり，さらに2か月後の右眼硝子体生検で悪性リンパ腫の診断が確定した。両眼にそれぞれ6回のメトトレキサート硝子体注入を行った。中枢神経と脊髄腔に悪性リンパ腫の所見はなかったが，メトトレキサートの髄腔内投与を行った。前房水の生検でIL-10が測定限界以下に低下していた。以後7か月後の現在まで経過は良好である。結論：眼内に原発した悪性リンパ腫に対し，メトトレキサートの硝子体注射と髄腔内注射を行い，診断から1年間の経過が良好であった。

Abstract.　Purpose：To report a case of primary intraocular malignant lymphoma treated by bilateral vitreal and preventive intraspinal injection of methotrexate. Case：A 69-year-old male presented with impaired vision in both eyes since one month before. Findings and Clinical Course：Corrected visual acuity was 0.7 right and 0.4 left. The anterior chamber was free of signs of inflammation. Both eyes showed vitreous opacity. Vitreous biopsy showed elevated values of interleukin（IL-）6 and 10 in the left eye 2 months later. Vitreous biopsy in the right eye led to the diagnosis of malignant lymphoma another 2 months later. Both eyes received vitreal injection of methotrexate in 6 sessions each. While no signs of malignancy were found in the central nervous system or cerebrospinal fluid, methotrexate injection was performed into the spinal fluid. The patient has been doing well for 7 months until present. Conclusion：Vitreal and preventive intraspinal injection of methotrexate was effective for primary intraocular malignant lymphoma for 12 months after diagnosis.
