要約　85歳の男性に両眼の白内障手術とハイドロジェル眼内レンズ挿入を行った。術後の視力は左右とも1.2であった。18か月後に矯正視力が左右とも0.6に低下した。後発白内障があり，これに対してNd：YAGレーザーを左右とも27発照射し後囊を円形に切開した。視力は右1.0，左1.2に改善した。その6か月後に左眼視力が再び0.3に低下した。眼内レンズ後面に半透明膜があり，前部硝子体膜とは離れていた。これにNd：YAGレーザーを照射して視力は1.2に改善した。右眼視力は良好であったが，水晶体上皮細胞が後囊の切開縁から中央部に向かって増殖伸展し，左眼と同様な半透明膜を部分的に形成していた。Nd：YAGレーザーによる後囊切開で生じた細胞成長因子が眼内レンズに取り込まれ，これが水晶体上皮細胞が増殖する原因になったと推定された。

Abstract.　A 85-year-old man received bilateral cataract surgery with implantation of hydrogel intraocular lens（IOL）. His visual acuity improved to 1.2 in either eye. Eighteen months later,the visual acuity decreased to 0.6 in either eye due to aftercataract. Both eyes were treated with 27 shots of Nd：YAG laser to create a circular opening. The visual acuity improved to 1.0 right and 1.2 left. Another 6 months later,his left visual acuity decreased to 0.3. A semi-opaque membrane had formed on the posterior surface of IOL. The membrane was separated from the anterior vitreous. Laser capsulotomy induced improvement of vision to 1.2. The right eye had full visual acuity,but the lens epithelial cells had started to proliferate and migrate centrally from the edge of capsulotomy opening. It appeared that laser capsulotomy released the epithelial growth factor which then accumulated in the hydrogel IOL,resulting in proliferation of lens epithelial cells as was observed in this patient.
