水晶体囊混濁のない水晶体囊内に挿入された眼内レンズが脱臼を繰り返した1例

要約　目的：囊内に挿入された眼内レンズ（IOL）が脱臼を繰り返し，最終的に水晶体囊ごとIOLを摘出した症例の報告。

症例と経過：34歳男性が他医で2年前に白内障手術を受け，当科で右眼に白内障手術を受けた。両眼とも水晶体囊の混濁はなく，IOLは囊内に固定され，偏位はなかった。幼少時から全身にアトピー性皮膚炎があり，眼周囲をこする癖があった。3年後に左眼の視力低下で受診した。IOLの支持部が虹彩前面に脱出して偏位し，Zinn小帯が部分的に断裂していた。IOL整復術が行われた。1か月後にIOLが再び脱臼し，光学部が大きなIOLと交換した。2年後にIOLが三度目の脱臼を起こし，IOLを水晶体囊とともに摘出した。摘出した水晶体囊には混濁はなかった。右眼には経過中に問題はなかった。

結論：後発白内障が囊内に挿入したIOLの固定に関与している可能性がある。

Abstract　Purpose: To report a case who showed repeated dislocation of in-the-bag inserted intraocular lens（IOL）needing eventual removal of IOL with the lens capsule.

Case and Clinical Course: A 34-year-old male received cataract surgery with IOL implantation in the left eye. He had received similar surgery in the right eye 2 years before. He had had atopic dermatitis since childhood and had the habit to rub his face. He sought medical repositioning of IOL. The IOL dislocated another one month later, needing implantation of IOL of larger size. IOL dislocated again 2 years later so that IOL was removed together with the lens capsule. The removed lens capsule showed no opacity or proliferation. The right eye has been doing well throughout.

Conclusion: The present case seems to suggest that proliferation of lens capsule is essential in the proper fixation of inserted IOL.
