白内障手術13,000眼のうち術後感染症を発症した2例の検討―手術室環境と手術時間が術野の汚染度に及ぼす影響

Two cases of postoperative infection in a consecutive 13,000 cases of cataract surgery.: Influence of cleanliness of surgical theater and duration of surgery

　1998年から2003年までの16年間に当院で白内障手術を行った13,024眼のうち，2眼に術後感染が起こった。1例は慢性涙囊炎がある72歳女性で，手術2週後に牽引糸をかけた6時の部位に潰瘍が起こり，角膜膿瘍から眼内炎に発展した。他の1例は72歳男性で，眼内レンズを毛様溝に縫着後に眼内炎が発症し，硝子体手術を行った。手術中の結膜囊培養結果は，手術室の清浄度の上昇と手術時間の短縮が術中の手術の汚染度を低下させ，術中の感染予防に有効であることを示していた。手術の数日後に結膜囊から採取された菌の過半数は，術前から存在していた菌と同種であったことから，角膜切開白内障手術では感染が術後早期に起こる危険性があると考えた。複数の感染発症因子が重なった場合には，より綿密な経過観察が必要であるが，基本的な感染予防対策を忠実に行えば，術後眼内炎のほとんどは予防が可能であると結論される。

Postoperative endophthalmitis developed in two cases out of a consecutive 13,024 cases of cataract surgery during 16 years through 2003. One was a 72-year-old female with chronic dacryocystitis. Two weeks after surgery,corneal ulcer formed at the site of control suture in the lower limbus that later developed to endophthalmitis. The other was a 72-year-old male. Endophthalmitis occurred after transscleral fixation of intraocular lens necessitating vitreous surgery. Results of intraoperative culture of conjunctival scraping showed that endophthalmitis can be prevented by better cleanliness of the surgical theater and by shortening the duration of surgery. The majority of microorganisms isolated from the conjunctival sac several days after surgery were the same as those present before surgery. Most cases of postoperative infection can be avoided by keeping the cornea free of wound,by selecting proper antibiotics before and after surgery,by keeping the surgical room clean,and by shortening the duration of surgery.
