Abstract 文献概要

　目的：Tension band wiring法は骨端部の筋，靱帯付着部の骨折に対する優れた骨接合法として広く普及している．しかし，内固定材による様々な合併症の問題は未解決であり，治療に難渋することも少なくない．そこでわれわれはpin-sleeve systemを用いた骨接合術を行ったところ良好な成績を得たので報告する．対象：2001年2月から2004年5月までの間に当科においてpin-sleeve systemを用いて手術を施行した症例は肘頭骨折4例，足関節果部骨折15例，および膝蓋骨骨折5例であった．結果：X線学的評価では全例骨癒合は良好に得られた．また，pinの逸脱は1例も認めなかった．結語：Pin-sleeve systemは有用なimplantと思われた．

　Tension band wire fixation is a popularly technique for various case of fracture, may, however, leads to a higher incidence of complications with fixation implants, due mainly to the backing out of the Kirschner wire or cutting off of soft wire. To avoid these problems, we used the pin-sleeve system substituting Kirschner wire and soft wire. We treated 24 cases with this technique―4 olecranon fractures, 15 malleolar ankle fractures and 5 patella fractures. Mean follow―was 7.7 months. All cases have been acquired completely fusion, and no backouts of the implant. The pin-sleeve system is thus beneficial for tension band wiring.
