鎖骨遠位端骨折に対し,肩鎖関節に干渉するtension band wiring 7肩と,干渉しないScorpion plate 11肩について臨床成績を比較検討した.骨癒合は全例に認められ,最終観察時肩関節可動域は両群に有意差がなかった.Tension band wiring群では肩鎖関節骨性変化が7肩中6肩に認められた.内固定具抜去前の疼痛(JOAスコア)はScorpion plate群が有意に少なかった.Scorpion plateは術後早期に社会復帰を望む患者にとって有用な固定方法と考えられた.
The purpose of this study was to compare the clinical results of transacromioclavicular tension band wiring (T group) and Scorpion plate fixation (S group) for distal clavicle fractures. There were 17 subjects (18 shoulders, 7 in the T group and 11 in the S group), and their mean age was 43.7 years (range, 19-81 years old). Mean follow-up time was 305.6 days. Bony fusion was achieved in every case. There was no significant difference in ROM of shoulder at the last visit. In 6 of the 7 shoulders of T group, osteoarticular change developed around the acromioclavicular joint, but no change occurred in the S group. There was significantly less pain in the S group than in the T group before extraction of the internal fixation.
