　症例は死亡時75歳女性。病初期から高度の球・仮性球麻痺を呈し，1年2カ月目に人工呼吸器装着。やがて核上性眼球運動障害を合併し，全経過2年10カ月で死亡。剖検では運動ニューロン系に加えて基底核や黒質，クラーク柱の背側神経核や脊髄小脳路にも高度の変性を認め，広汎型筋萎縮性側索硬化症と診断した。さらに，高度の索変性を呈した皮質脊髄路と脊髄小脳路を除く脊髄白質全域に，マクロファージの軽度の浸潤を伴う空胞状変化を認め，特に頸髄楔状束は対称性に高度の空胞で充満していた。同病変については炎症細胞浸潤や選択的な病変分布から死後の人工産物とは考えにくく，原疾患の末期に合併した変化と考えられた。空胞性脊髄症の既報告例には，本例のような楔状束に最も高度であるという分布を呈した症例はなく，本例に特徴的である。

We report on vacuolar myelopathy in a case of spo-radic amyotrophic lateral sclerosis (ALS) with multi-systemic degeneration. The patient was a 75-year-old woman at death, who had been characterized by se-vere upper and lower motor neuron disturbance and supranuclear ophthalmoplegia. She became dependent on a respirator 14 months and survived 34 months af-ter the onset of her illness. Neuropathologically, se-vere degeneration of the basal ganglia, substantia ni-gra, Clark's dorsal nucleus and spinocerebellar tract were noted together with the prominent alteration of the motor neuron system.
