We report a 78-year-old female presenting fluctua-tion of cerebral ischemic symptoms for a long time. Three weeks after transient ischemic attack of speech disturbance, she suffered right hemiparesis and motor dominant aphasia. The symptoms gradually pro-gressed. Magnetic resonance angiography (MRA) showed a stenosis of branches of the left middle cere-bral artery and single photon emission computed tomography (SPECT) did hypoperfusion of the same artery territory. Though antiplatelet and anticoagulant therapy was continued, the symptoms fluctuated.
