Abstract 文献概要

　症例は29歳男性。感冒症状の3日後，意識障害と全身性けいれん重積状態（GSE）を呈して発症。抗けいれん剤の効果がなく，全身麻酔薬midazolamとpropofolのほか，vecuronium bromideを大量持続静注したにもかかわらずGSEは抑制不可能であり，脳浮腫が進行して第10病日に脳死状態に陥った。その直後からGSEは完全に消失し，第16病日に死亡した。髄液検査で細胞増多や蛋白上昇を認め，臨床的にはウイルス性脳炎と診断した。しかし，剖検では脳炎所見はなく，脳ヘルニアを伴う高度の脳浮腫と無酸素性脳症，島回に限局した軽度のグリオーシスを認めたのみであった。本例の急性浮腫性脳症は，一種のacute toxic encephalo-pathyと考えられる。本例では，全身静脈麻酔でも抑制不能な極めて難治性のGSEが脳死に至るまで持続し，浮腫性脳症の治療を考えるうえで臨床的に貴重な症例と思われる。

A previously healthy, 29-year-old man developed generalized convulsion with disturbed consciousness 3 days after the flu-like symptoms. On the next day, his convulsion evolved into generalized status epilepticus (GSE) that was unresponsive to administration of vari-ous anticonvulsants. Then, intravenous anesthetic agents, including midazolam, propofol and vecuronium sodium, were continuously infused. During the follow-ing 9 days, despite of high-dose infusion of these an-esthetic agents, his GSE could not be suppressed, and complex partial seizure with secondary generalization frequently occurred.
