　症例は68歳男性。上気道炎症状の後，咽頭筋麻輝から始まり，続いて失調性歩行，両側上肢異常感覚，外眼筋麻痺が出現した。深部腱反射の消失も認められMiller Fisher症候群と診断され，血漿交換が施行された。急性期の血清でIgG抗GQ1b抗体価とIgG抗GT1a抗体価の上昇が認められ，後者は前者より高値であった。近年，咽頭筋麻痺と抗GT1a抗体活性の関連が報告されている。Fisher症候群で咽頭筋麻痺が初発症状となるのは稀であるが，本例は抗GT1a抗体価が抗GQ1b抗体価に比べて高かったことが，初発症状としての咽頭筋麻痺の出現に関連すると考えられた。

We report herein a rare case of Miller Fisher syn-drome with pharyngeal palsy as an initial symptom. A68-year-old man admitted to our hospital with pharyn-geal palsy two weeks after a respiratory infection. Hesubsequently developed ataxic gait, paresthesia in theupper limbs and ophthalmoplegia. Double-filtrated -plasmapheresis had been performed four times and allthe symptoms subsided within two months. In theacute phase of the disease, the titers of anti-GQ 1 band GT 1 a antibodies were elevated. The titer of anti-GT 1 a antibody was higher than that of anti-GQ 1 bantibody.
