　症例は29歳男性。1999年7月から発汗過多，下痢，食欲亢進，精神的緊張・興奮を自覚し，8月から車の運転中や椅子などに座って同じ姿勢を保持していると両下肢が硬直するようになり，2000年1月入院となった。一般身体所見ではびまん性の甲状腺腫があり，神経学的所見では近位筋に筋力低下を認める以外正常であった。血液生化学検査ではfree T3とfree T4は高値であり，TSHは低値と甲状腺機能亢進を認め，TSHレセプター抗体は高値であった。下肢のMRIでは両側大腿二頭筋で筋萎縮を認めた。入院後メチマゾール1日30mg処方したところ，同じ姿勢を保っても筋の硬直はなく，筋力も徐々に改善した。考察では本症例の臨床的特徴と共に鑑別疾患について述べた。

We report a patient with thyrotoxic myopathy asso-ciated with unusual muscle symptoms. A 29-year-oldman developed hyperhidrosis, diarrhea, increase in ap-petite, and excitability in July, 1999. In August, he ex-perienced muscle stiffness in bilateral lower extremi-ties after maintaining postures such as driving a car orsitting on a chair. He was admitted to our hospital, inJanuary, 2000. On physical examination, goiter wasnoted. Neurological examination was normal exceptfor proximal muscle weakness . Laboratory testshowed elevated free T 3 and free T 4, decreasedTSH.
