静脈性血管腫を合併した脳動静脈奇形の1例

A Case of Arteriovenous Malformation Associated with Venous Malformation

　患者は35歳男性。突然の頭痛にて発症し，頭部CTにて左後頭葉の脳出血と急性硬膜下血腫が認められた。脳血管撮影にて脳動静脈奇形を認め，血腫除去および脳動静脈奇形摘出を行った。また，脳動静脈奇形とは離れた部位である左側頭葉に静脈性血管腫が認められ，経過観察中である。近年，脳動静脈奇形，静脈性血管腫，海綿状血管腫，毛細血管拡張症の4種の脳血管奇形には互いに合併型があることが分かってきているが，比較的珍しい症例であるため報告する。

A case of arteriovenous malformation associatedwith venous malformation is reported. A 35-year-oldmale patient suffered intracerebral hemorrhage in theleft occipital with acute subdural hematoma. Angiogra-phy showed an arteriovenous malformation fed by theleft middle cerebral artery. The hematoma and niduswas operatively removed. He also had a venous mal-formation in the left temporal lobe. This rare case isreported with some discussion.
