　慢性脳循環不全症患者24例を対象にシロスタゾールを平均209日間投与し，脳血流量並びに臨床症状に対する有効性と安全性について検討を行った。脳血流の検討はPatlak plot法を用い定量的に行った。自覚症状全般改善度における中等度改善以上の割合（改善度）は最終で52.2％であり，症状別ではdizziness，立ちくらみ，頭痛と頭重感に30％以上の改善度を認めた。本剤投与により大脳半球および小脳半球のrCBFは有意に増加した。また自覚症状全般改善度と大脳半球を除く各部位のΔ％rCBFとの間に正相関を認めたが，精神症候全般改善度とは正相関を認めなかった。症状別ではめまいの改善度と左側頭葉，両基底核，両小脳半球のΔ％rCBFが0.7以上の正相関を示した。副作用は頭痛が8例に発現したが，減量または投与中止後，消失を認めており，その他問題となる副作用は特に認めなかった。以上，本剤は慢性脳循環不全症の治療薬として有用であると考えた。

This report deals with a study of the effectiveness,safety, and usefulness of long-term administration of Cilostazol for the improvement of cerebral blood flow and clinical symptoms in 24 patients with Chronic Cerebral Circulatory Insufficiency. Cerebral blood flow was investigated quantitatively using the Patlak plot method. Cilostazol was orally adminis-tered for 209 days on average. In the global improve-ment rating assessed on the basis of all subjective symptoms, the final improvement rate, comprising all cases showing moderate or better improvement,was 52.2%.
