　頭蓋内テント上占拠性病変が原因となり脳幹が偏位し，対側大脳脚がテント自由縁により圧排され障害を受けて，病変と同側に片麻痺が起こる偽性局在徴候はKernohan's notchと称され臨床的，病理学的によく知られているが，画像で確認された報告は少ない。一方，慢性硬膜下血腫において脳ヘルニア症状を呈するほどの症例は比較的稀である。症例は43歳男性で，脳ヘルニア症状を呈した左慢性硬膜下血腫症例に対して穿頭術を施行して治療したが，左片麻痺が残ったためMRIにて精査したところKernohan's notchを確認し得た。さらに，脳幹部のsurface imageをT₁強調画像coronal viewのthin slice画像を合成して作り上げた3次元MRI画像にて，Kernohan's notchのsurface imageが鮮明に描出された。脳幹病変が疑われる症例に対して画像構成に工夫を加えたMRI検査を積極的に行っていくことが肝要と思われる。

Compression of the crus cerebri against the free edge of the tentorium contralateral to a supratentor-ial mass, the so-called Kernohan's notch, can be a cause of false localizing sign. Kernohan's notch has been thoroughly studied clinically and pathological-ly, but not radiographically. The authors describe acase of left chronic subdural hematoma, which resulted in left hemiparesis caused by Kernohan's notch. Injury to the contralateral cerebral peduncle was clearly shown by magnetic resonance imaging (MRI) performed in the postoperative period.
