肥大型心筋症における心電図所見の推移—5年以上経過観察中の50例での検討

Clinical Features and Electrocardiographic Changes in Long-Term Follow-up of 50 Patients with Hypertrophic Cardiomyopathy

　5年以上（平均7.6年間）観察可能であった肥大型心筋症50例を対象に臨床状態悪化の有無別に心電図変化を比較検討した．臨床状態悪化群20例では不変群30例に比し，R波振幅の減高，ORS時間の延長，P波左房成分の増大，異常Q波の出現頻度が有意に高かった．これらの心電図所見に注目することは本症の予後判定に有用であると思われた．

We studied the relation of ECG changes during the clinical course in patients with hypertrophic car-diomyopathy (HCM). Fifty patients with HCM were categorized in two groups; 20 patients with, and 30 patients without signs of deterioration of clinical state. The changes between the first and the final ECG were compared in these two groups. The average follow-up period was 7.6 years.

Twenty patients with clinical deterioration presented increase in QRS intervals (0.06→0 0.071sec, p＜0.025), decrease in voltage of RV₅(2.81→2.38sec, p＜0.025), increase in newly developed abnormal Q waves, and P wave changes.
