　心房中隔欠損症は，成人にみられる先天性心疾患の中では最も頻度の高い疾患の1つである。ところが，このうち冠状静脈洞型心房中隔欠損症（coronary sinus ASD）は，比較的まれな疾患である。今回我々は，64歳女性のcoronary sinus ASDで，左冠状動脈前下行枝に有意な狭窄を有する症例に対して，ASD閉鎖と同時に，大動脈冠状動脈バイパス術（A-Cバイパス術）を施行し，良好な結果を得たので，若干の考察を加えて報告する。

A 64-year-old patient with coronary sinus ASD and significant left anterior descending artery stenosis was successfuly treated by concomittant ASD patch closure and aortocoronary bypass procedure. The defect was closed with Teflon patch so as to drain coronary sinus blood into the right atrium. We have not encountered conduction abnormalities after repair. I think that the A-V node is in the nomal location in the coronary sinus ASD. So this is a better procedure physiologically and surgically. To the best of our knowledge, this is the oldest surgical case of coronary sinus ASD.
