開心術後の呼吸管理におけるIntermittent Assisted Ventilation（間歇的補助呼吸）の応用

Intermittent assisted ventilation in respiratory management after cardiac surgery

　最近の心臓外科の発展は，重症例をも外科治療の対象にしてきており，術中管理はもとより術後管理は更に重要な位置を占めてきている。なかでも呼吸機能と循環機能とは密接な関係にあるため，術後の呼吸管理には細心の注意が必要である。

　今回，我々はBennett MA−1と1台のローラーポンプを使用して自発呼吸と人工呼吸器による補助呼吸とが混在する呼吸パターン――間歇的補助呼吸――の可能な回路（Intermittent Assisted Ventilation回路，以後IAV回路と略す）を工夫し，開心術後の人工呼吸器よりのWeaning（離脱〉の際に応用し良好な結果を得た。

　Intermittent Assisted Ventilation (I. A. V.) is a method included spontaneous breathing and assisted ventilation. The I. A. V. apparatus consists of respirator and constant flow system, using respirator (Bennett MA-1), O₂ Blender (Bourns) and Mini-Pump (Tonokura).

　The roller-pump is used to stop constant flow intermittently and assisted ventilation is performed by Bennett MA-1. I. A. V.
