Supernormal Conduction and Excitation Observed in Patients Treated with an Implantable Pacemaker

　植込式心臓ペースメーカーが諸外国^6）10），わが国^28）で用いられるようになってから約10年を経過する。この間，以前カエルやイヌなどの動物において認められていた電気生理学上の諸現象，たとえば絶対不応期，相対不応期，細動受攻期（vulnerable period），過常期^1），Wedensky現象^17）32）などが，ヒトの心臓でも実際に確められるようになってきた。

　過常期（supernormal phase）は相対不応期の終りにおける一過性に興奮性の高まる時期をさす。興奮性にはexcitability　（被刺激性）とconductivity　（伝導性）の2つの面がある。より弱い刺激で反応が起これば被刺激性が高いとされる。より容易に，またはより早く興奮が伝導されれば伝導性がよいとされる。両者とも興奮性が高まった状態である。過常期にも，この両者に相当する場合があり，この時期においてその前後の時期にくらべて弱い刺激で反応する過常期興奮（supernormal excita—bility）の場合と興奮の伝導が速く容易になる過常期伝導（supernormal conductivity）の場合とがあり，両者を含めて過常性（supernormality）と呼ぶこともできる。

Among seventy-seven patients treated with a demand pacemaker, two patients showed evidence of a supernormal phase of A-V conduction. In one patient, supernormal conduction was noticed shortly after implantation of a pacemaker. The supernormal period ranged from 0.12 sec to 0.32 sec after Q wave. Seventy percent of P waves were conducted to the ventricle during the period from 0.26sec to 0.32sec after Q wave. In the other, a supernormal conductivity appeared four-teen months after implantation. The supernormal period ranged from 0.38sec to 0.42sec after Q wave. One month later, sinus rhythm was re-stored.
