ペースメーカー植込み後，閾値上昇により過常期興奮を生じた1例

Sunernormal Excitation due to Elevation of Ventricular Stimulation Threshold--Report of a Case

　ペースメーカー植込み後，一定期間，心筋刺激閾値は上昇する^1）が，これは心筋に接している電極先端周囲に線維化が生ずることが主な原因である。われわれは心内膜電極挿入後，著明な心筋刺激閾値の上昇を認め，心電図上，過常期興奮Supernormal Excitationを認めた症例を経験したので報告する。また過常期興奮が生ずる電流の強さとペーシング調律が生ずる電流の強さについて考案する。

Forty-seven year old female had a pacemaker implant because of bradycardia-tachycardia syndrome. Pacing failure started on the 2nd postoperative day, which became more obvious on successive days in spite of good demand mechanism.

Ventricular stimulation threshold must have increased. The position of endocardial pacing electrode seemed to be ideal on X-ray.

The fixed rate (72/min.) was obtained by the use of magnet, which demonstrated supernormal excitation.
