要旨●EBVはヘルペスウイルス科に属する2本鎖DNAウイルスで，成人の大部分が潜伏感染し，多彩な悪性腫瘍の発生に関連している．胃に腫瘍を形成するものでは，EBV関連胃癌や悪性リンパ腫のうちEBV陽性びまん性大細胞型B細胞リンパ腫，非特定型，節外性NK/T細胞リンパ腫，鼻型，慢性活動性EBV関連リンパ増殖病変，免疫不全関連リンパ増殖異常症などが認められる．EBV関連胃癌はリンパ球浸潤癌の特殊な組織像をとり，良好な予後に反映される．EBV関連の悪性リンパ腫は，大部分が免疫不全状態により発生するものであり，肉眼形態や病理組織学的所見も非定型的なものが多く，診断に難渋するものが多いため，病態の把握が必要である．

　EBV（Epstein-Barr virus）, a member of the herpes virus family, infects most of the adult population latently. EBV infection has been associated with the pathogenesis and tumor progression of various malignant neoplasms. The histologic types that form a mass in the stomach are EBV-associated gastric carcinoma, EBV-positive diffuse large B-cell lymphoma, other non-specified, extranodal NK/T cell lymphoma, nasal type, chronic active EBV infection, and immunodeficiency-associated lymphoproliferative disorders. EBV-associated gastric carcinoma has a special form of carcinoma with lymphoid stroma, which contributes to a good prognosis. Most EBV-associated lymphomas are caused by immunodeficiencies. They are macroscopically and histologically atypical and are difficult to diagnose. It is necessary to understand their pathogenesis.
