要旨●過敏性腸症候群（IBS）は機能性消化管障害の一つであり，“腹痛あるいは腹部不快感とそれに関連する便通異常が慢性もしくは再発性に持続する状態”と定義されている．またIBSの一部で感染性腸炎後に発症する群（PI-IBS）や小腸内細菌異常増殖（SIBO）がみられる群が指摘されている．腸管感染症やSIBOは，客観的に感染病原体や増殖細菌を確認することで診断される診断名であるが，IBSは主に症状から診断される症候群であり，病名に少々混乱があると思われる．いずれにしてもIBSの一部に，PI-IBSやSIBOを合併するIBSがあることは明らかで，今後これらの病態を区別しながら，新たな概念の構築や治療法の開発が望まれる．

　IBS（irritable bowel syndrome）is a representative functional gastrointestinal disorder and is characterized by chronic or recurrent abdominal pain and/or abdominal discomfort associated with abnormal bowel movement. PI-IBS（post-infectious-IBS）and IBS with SIBO（small intestinal bacterial overgrowth）are types of IBS. Although gastrointestinal infection and SIBO are diagnosed by detecting the causative pathogen and bacterial overgrowth, IBS is diagnosed based on its symptoms. Therefore, confusion regarding the name of the condition may exist. However, PI-IBS and IBS with SIBO exist, and new criteria and treatments should be developed by differentiating these entities.
