要旨●患者は70歳代後半，男性．臍部腫瘤の精査にて皮膚科を紹介され受診した．臍部に大きさ約20mm，暗赤色，弾性硬の結節状腫瘤を認めた．臍部皮膚生検を施行し，粘液産生を伴う高分化腺癌組織を認めた．サイトケラチンおよび粘液形質の免疫組織化学的検索に基づき，消化器癌の臍部転移を考え，腹部造影CTと上部消化管内視鏡を施行したところ，胃体上部大彎に1型進行胃癌を認めた．胃生検では臍部腫瘍と同様の高分化腺癌を認め，免疫組織化学染色の特徴も類似していたことより，進行胃癌の臍部転移（Sister Mary Joseph's Nodule）と最終診断した．約2年の比較的長期にわたり全身化学療法を継続し，診断から約25か月後に原病死した．

　A 70-year-old man was admitted to the department of dermatology in our hospital with an umbilical tumor. Biopsy of the lesion revealed a well-differentiated adenocarcinoma including mucinous components, with immunohistochemical findings of cytokeratin and mucin. Contrast-enhanced abdominal computed tomography and gastrointestinal endoscopy were performed to examine the primary tumor. Finally, gastrointestinal endoscopy revealed advanced gastric cancer. The umbilical metastasis in such a case is referred to as a Sister Mary Joseph's nodule. The patient underwent several combination chemotherapies for 24 months but died 25 months after presentation.
